If you don’t see your insurance provider listed here, don’t worry! We’ll file your insurance for you, but we do require full payment at the time of your visit. This way, your insurance provider will reimburse you directly for your visit to our office.
Payment is due in full at each appointment for services provided. Please be aware that the parent/guardian bringing the patient to our office is legally responsible for payment of all charges. For your convenience we accept cash, personal checks, Visa, Mastercard, Discover, American Express and CareCredit. Health Savings Accounts can be used for doctor services and optical goods. If you have insurance, please remember to bring your insurance card with you to every visit. Payment is due at time of service for all estimated portions of charges, deductibles, co-pay amounts and non-covered services. If your insurance company has not paid within forty-five (45) days, your balance is due in full.
